1. Payment is due in full at the time of the visit.
2. Payment methods include money order, Visa, MasterCard and Discover.
Cardholder must be present with photo identification.
3. We do not accept cash or personal checks.
4. We do accept most insurance plans and we are contracted with
the following insurance companies:
