Pre-Op Instructions

1. Please provide a complete medical history, including medications, when scheduling the appointment. If you forget to mention any medical problems, please call back immediately. Failure to disclose medical information prior to the appointment may result in being rescheduled. Do not take any medicines before the procedure unless you are otherwise instructed.
2. Plan to have a follow-up appointment 2 to 3 weeks after the procedure. Bring information to your appointment about where you will have your follow-up exam.
3. Do not eat, drink or smoke anything after midnight preceding your appointment. This includes gum, water, and tic-tacs. You may brush your teeth but spit out all of the water and do not swallow.
4. All patients receiving anesthesia other than local anesthesia must arrive with a ride who must then sign a release form upon registration. That person must remain in the waiting room for the duration of the appointment. If you are taking a taxi, you must have an adult escort. Only one person is allowed in the office with you, but they may not enter the medical area. Please do not bring any children into the office.
5. Wear or bring a pair of socks and underwear that can support a pad and dress comfortably.
6. You will be here for 3 to 4 hours. Please arrive on time. Arriving earlier or later will only increase your wait.
7. Please do not bring any large bags or brief cases. There is a security guard who will perform a scan with a metal detector.
8. You must bring photo identification showing your date of birth. If you are using insurance, please bring your insurance card with you.
9. Anyone under the age of 18 must have a full birth certificate with a seal. A legal guardian must be with you and have photo identification. The name on your guardian's ID must match the name on the birth certificate. If their name is different, legal documentation, such as marriage or divorce certificates, must be provided.
